A case of spermatic granuloma in the epididymis developed in 40 years old Korean after vasectomy-is reported. This granuloma, the reaction that results from the invasion of sperms into the stroma of epididymis, occurs most frequently during the decades of highest sexual activity, and is not rare as the relatively few reports in the literature might indicate. Clinically, the possibility of spermatic granuloma should be considered in differential diagnosis of epididymal mass because vasectomy is recently performed widely in this country as a mean of family plan and this surgical procedure is considered one of causative factors in the development of the granuloma.
